New Patient Center

Welcome to Florida East Coast Medical Group! Now that you’ve called, spoken to our friendly staff, and made your appointment, here are the next steps:

Make your life more convenient! Download the three new-patient forms and fill them out at home where you have all the information you need at your fingertips. Don’t forget to bring the forms with you to your appointment. The three forms are found below:

New Patient Form
Authorization for the Release of Medical Records
Privacy Policy and HIPAA Acknowledgement

On the day of your appointment, please bring the following items with you:

Your new patient forms.

Your driver’s license, state ID card, student ID card, military ID card, passport or any other official picture ID.

Your insurance card and/or Medicare card, and Medicare supplemental policy card (if applicable). If you choose to bring a photocopy or photograph of your insurance card, please ensure you include both the front and back of the card. Both sides have information we need!

Please bring a complete list of your medications, including the name of the drug, dosage, and how often you take the medication. Or better yet, bring the bottles with you!

Finally, bring your biggest smile! It makes our day.
